{"id":59983,"date":"2018-01-19T20:23:26","date_gmt":"2018-01-19T20:23:26","guid":{"rendered":"http:\/\/www.bnreport.com\/brexit-shockwaves-hit-the-nhs-5\/"},"modified":"2018-01-19T20:23:26","modified_gmt":"2018-01-19T20:23:26","slug":"brexit-shockwaves-hit-the-nhs-5","status":"publish","type":"post","link":"https:\/\/www.bnreport.com\/en\/brexit-shockwaves-hit-the-nhs-5\/","title":{"rendered":"Brexit shockwaves hit the NHS"},"content":{"rendered":"<p>LONDON \u2014 After Brexit, prepare for the next U.K. revolution \u2014 in Britain\u2019s cherished public healthcare system.<\/p>\n<p>With health leaders this winter all but admitting their inability to cope \u2014 making an unprecedented call for hospitals to postpone all non-urgent operations in January \u2014 momentum is building for major reform of the country\u2019s \u00a3125 billion-a-year National Health Service.<\/p>\n<p>The NHS has been struggling to reconcile increased demand from an aging population with tightened public spending for years and calls for an overhaul aren\u2019t directly linked with Brexit. But for some policymakers, the U.K.\u2019s departure from the EU is serving as a moment for self-reflection: to examine what the British truly value.<\/p>\n<p>One of the institutions in the spotlight is the NHS.<\/p>\n<p>\u201cTo heal the divisions of the Brexit referendum it is vital that we recast Brexit as an inspiring moment of electrifying and comprehensive national renewal \u2014 economic, cultural, political,\u201d said George Freeman, chair of the Conservative Party\u2019s policy forum. He believes it can be a moment for \u201cre-energizing our approach to domestic issues even if they have nothing to do with Europe\u201d \u2014 including the NHS and social care.<\/p>\n<p>\u201cThe closest thing the British have to religion is the national health care system,\u201d said Robert Colvile, director of the Centre for Policy Studies, a British think tank that has called for a cross-party government advisory committee to look into revamping the NHS.<\/p>\n<p>\u201cOne of the powerful things about Brexit is it forces you to look at yourself in the mirror,\u201d he added. \u201cEven if Brexit and the NHS don\u2019t have [much] to do with each other, you don\u2019t have any excuses anymore. After Brexit, they [the problems] are your fault; it\u2019s up to you to fix them.\u201d<\/p>\n<h3>Spending restraint<\/h3>\n<p>Prime Minister Theresa May has precious little time or political capital for a major reform of the NHS funding model. But a growing chorus of her political allies say there could be no project more likely to galvanize a divided British population than saving the NHS.<\/p>\n<p>Currently funded almost entirely from taxation, the NHS stands apart from healthcare systems in most rich European countries where services are more often funded through a mix of the state and the individual, via personal or employee insurance.<\/p>\n<p>But after years of spending restraint since the 2008 economic downturn, and with Brexit set to put a dampener on future economic growth, policymakers are beginning to question how long the current system can last without major new streams of funding.<\/p>\n<blockquote>\n<p>\u201cThe prime minister also needs to set out more clearly why Brexit will be a moment of domestic renewal\u201d <em>\u2014 George Freeman, chair of the Conservative party\u2019s policy forum<\/em><\/p>\n<\/blockquote>\n<p>Calls for a cross-party commission to consider how to fund the NHS and the country\u2019s struggling care system were given a huge boost last week when Theresa May\u2019s former chief of staff, Nick Timothy (who still has the ear of the prime minister), backed the idea, <a href=\"http:\/\/www.telegraph.co.uk\/news\/2018\/01\/03\/coalitions-nhs-reforms-have-proved-disastrous-need-royal-commission\/\" rel=\"noreferrer\" target=\"_blank\">endorsing a tax increase on the over 40s<\/a> to cover some of the funding gap.<\/p>\n<p>Norman Lamb, a former health minister in the coalition government of 2010 to 2015, who has long backed a cross-party commission, said Brexit isn\u2019t the linchpin for overhauling the NHS, but it does impact the NHS in major ways.<\/p>\n<p>The first is money.<\/p>\n<p>Brexiteers campaigned on the idea that leaving the EU would provide the U.K. with extra money that could be injected into the NHS \u2014 the notorious and discredited \u00a3350 million a week claim on the side of their battle bus. The real impact of Brexit is likely to be less money, said Lamb. If Brexit \u201cundermines our economic performance, then it undermines our ability to fund our public services, including the NHS,\u201d he said.<\/p>\n<p>Second is staffing. <a href=\"https:\/\/www.theguardian.com\/society\/2017\/nov\/02\/european-nurses-midwives-leaving-uk-nhs-brexit-vote\" rel=\"noreferrer\" target=\"_blank\">Figures released<\/a> by the U.K.\u2019s Nursing and Midwifery Council in November suggest that the number of EU nurses and midwives coming to work in Britain from Europe has dropped by 89 percent since the referendum. A significant proportion of NHS stuff \u2014 <a href=\"http:\/\/researchbriefings.parliament.uk\/ResearchBriefing\/Summary\/CBP-7783\" rel=\"noreferrer\" target=\"_blank\">62,000 or 5.6 percent<\/a> \u2014 are EU27 nationals so if the U.K. continues to be a less attractive location for healthcare workers as Brexit unfolds, that could spell serious staffing problems for the NHS.<\/p>\n<p>Despite the problems, Freeman, who was formerly head of May\u2019s Downing Street policy board and is now the fulcrum between Tory policymaking and grassroots activists, told POLITICO that big new ideas on the NHS are an opportunity to \u201ccarry with us\u201d those who fear Brexit will be a distraction from issues they care about.<\/p>\n<p>\u201cWhilst ministers negotiate Brexit over the next 15 months, the prime minister also needs to set out more clearly why Brexit will be a moment of domestic renewal,\u201d he said.<\/p>\n<p>In the same way that Michael Gove, the environment secretary and prominent Leave campaigner, has won plaudits for his <a href=\"https:\/\/www.politico.eu\/article\/michael-gove-sets-out-green-brexit-farming-subsidy-reform\/\" rel=\"noreferrer\" target=\"_blank\">promise of a \u201cgreen Brexit,\u201d<\/a> May could revitalize the Tory brand by championing an NHS and care revolution, Freeman believes.<\/p>\n<p> <img decoding=\"async\" src=\"https:\/\/www.politico.eu\/wp-content\/uploads\/2018\/01\/GettyImages-521388664-714x475.jpg\"\/><\/p>\n<p>Brexiteers campaigned on the idea that leaving the EU would provide the U.K. with extra money that could be injected into the NHS | Oli Scarff\/AFP via Getty Images<\/p>\n<p>Contrary to more traditional strands of Conservative thought, that doesn\u2019t mean boosting NHS internal market forces further, he says, but taking lessons from European neighbors on alternative funding models \u2014 including the highly controversial idea (in the U.K.) of co-payments by individuals.<\/p>\n<p>\u201cWe need to be less parochial and look at how other countries fund their healthcare systems. With an aging population we should aspire to be amongst the world\u2019s leaders in health spending as a percent of GDP \u2026 Nobody wants an American style insurance-based system \u2026 but there\u2019s a huge range of different models across Europe which we should look at in designing the right system for a modern U.K.\u201d<\/p>\n<h3><strong>Crystal ball<\/strong><\/h3>\n<p>With the NHS held in high public esteem, such a reform would prove very politically challenging, and Freeman agrees with Timothy and Lamb\u2019s call for a cross-party approach.<\/p>\n<p>Backing from the likes of Timothy makes it all the more likely that May will give the green light to some kind of review. But also key will be the Health and Social Care Secretary Jeremy Hunt, who was reappointed with an expanded brief in<a href=\"https:\/\/www.politico.eu\/article\/theresa-mays-reshuffle-that-wasnt\/\" rel=\"noreferrer\" target=\"_blank\"> this week\u2019s Cabinet reshuffle<\/a>.<\/p>\n<p>Hunt will also take over responsibility for a major reform of social care, due in the summer, which was going to be led by May\u2019s former deputy Damian Green, before his resignation before Christmas.<\/p>\n<p> <img decoding=\"async\" src=\"https:\/\/www.politico.eu\/wp-content\/uploads\/2017\/07\/GettyImages-801768472-714x476.jpg\"\/><\/p>\n<p>Britain\u2019s Health and Social Care Secretary Jeremy Hunt | Daniel Leal-Olivas\/AFP via Getty Images<\/p>\n<p>Senior figures, including Tory MP Sarah Wollaston, chair of the House of Commons health select committee, believe that a review that looks just at care, and not health, won\u2019t get to the heart of the matter. \u201c[It] will miss the point that these two systems cannot be considered in isolation,\u201d she wrote in the <a href=\"https:\/\/www.ft.com\/content\/3ce98150-f240-11e7-bb7d-c3edfe974e9f\" rel=\"noreferrer\" target=\"_blank\">Financial Times<\/a>.<\/p>\n<p>Hunt seems to agree. \u201cHow we treat the elderly is the litmus test of a civilised [sic] society,\u201d <a href=\"https:\/\/twitter.com\/Jeremy_Hunt\/status\/950726343368626178\" rel=\"noreferrer\" target=\"_blank\">he tweeted Tuesday,<\/a> \u201cThe health and social care systems are umbilically linked, so putting leadership in one govt department makes sense as a first step ahead of a vital green paper.\u201d<\/p>\n<p>Tipped for promotion and widely thought to have leadership ambitions, Hunt fought against being moved on from the health brief by May, according to <a href=\"https:\/\/www.thetimes.co.uk\/edition\/news\/defiance-and-derision-greet-theresa-may-s-reshuffle-day-of-mixed-messages-837cwwfld\" rel=\"noreferrer\" target=\"_blank\">reports<\/a> of her Cabinet reshuffle. If there is to be a major push to revitalize the NHS, he may fancy himself as the man to lead it.<\/p>\n<p>A vocal advocate of increasing the NHS budget, Hunt <a href=\"http:\/\/www.independent.co.uk\/life-style\/health-and-families\/health-news\/a-huge-effort-is-required-to-keep-nhs-fully-funded-by-the-taxpayer-says-jeremy-hunt-10394226.html\" rel=\"noreferrer\" target=\"_blank\">said in 2015<\/a> that while he was \u201cconfident\u201d the service can remain taxpayer funded for the next generation, he did not have a \u201ccrystal ball.\u201d<\/p>\n<p>That was before Brexit.
