{"id":151702,"date":"2020-10-20T14:50:05","date_gmt":"2020-10-20T14:50:05","guid":{"rendered":"https:\/\/www.bnreport.com\/the-hiv-fight-is-growing-old\/"},"modified":"2020-10-20T14:50:05","modified_gmt":"2020-10-20T14:50:05","slug":"the-hiv-fight-is-growing-old","status":"publish","type":"post","link":"https:\/\/www.bnreport.com\/en\/the-hiv-fight-is-growing-old\/","title":{"rendered":"The HIV fight is growing old"},"content":{"rendered":"<p><em>This article is part of <a href=\"https:\/\/www.politico.eu\/telescope-hub\/\" rel=\"noreferrer noopener\" target=\"_blank\">Telescope: The New AIDS Epidemic<\/a>, a deep-dive investigation into the modern face of a disease that transformed the world.<\/em><\/p>\n<p>As medical breakthroughs allow more and more people to grow old with AIDS, the virus has \u2014 in many circles \u2014 become old news.<\/p>\n<p>Few organizations know this better than the Austrian charity <a href=\"https:\/\/www.diversitycare.wien\/\" rel=\"noreferrer noopener\" target=\"_blank\">Diversity Care Vienna<\/a>. Since its founding in 1999, the unassuming NGO \u2014 which provides home nursing to AIDS patients in Vienna \u2014 could count on a donation of around \u20ac100,000 a year via Austria\u2019s <a href=\"https:\/\/lifeplus.org\/en\/history-draft-eng\/\" rel=\"noreferrer noopener\" target=\"_blank\">Life Ball<\/a>, a glitzy, raunchy gala to raise cash for the fight against HIV. It was, until recently, the biggest charity event for the cause in Europe. Bill Clinton was a regular, as were Elton John and designers like Jean Paul Gaultier and Vivienne Westwood.<\/p>\n<p>Yet in 2016, the check dropped to \u20ac75,000. In 2017, it was down to \u20ac50,000, and then \u20ac25,000 in 2018. That turned out to be its parting gift. Last year, Life Ball\u2019s organizers announced that 2019 would be the final party, after 26 years, leaving Diversity Care deep in the red, hanging on largely thanks to the help from the city.<\/p>\n<p>\u201cAIDS has changed from a death sentence to being a chronic disease,\u201d said Life Ball founder Gery Keszler, <a href=\"https:\/\/lifeplus.org\/en\/\" rel=\"noreferrer noopener\" target=\"_blank\">explaining his decision<\/a> to shut it down last year. \u201cThe paradox of this success is that the number of allies for AIDS charity projects is decreasing both at home and abroad.\u201d<\/p>\n<p>Diversity Care\u2019s travails are representative of the new challenge facing organizations around the world fighting AIDS.<\/p>\n<p>The Global Fund to Fight AIDS, Tuberculosis and Malaria <a href=\"https:\/\/www.theglobalfund.org\/media\/10103\/corporate_2020resultsreport_report_en.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">estimates<\/a> a 30 percent shortfall in what\u2019s needed to respond to HIV in poor countries this year \u2014 not even taking into account COVID-19, which is straining national budgets.<\/p>\n<p>Donations for the global fight against HIV from wealthy countries were down $200 million in 2019 compared to the previous year, and <a href=\"http:\/\/files.kff.org\/attachment\/Donor-Government-Funding-for-HIV-in-Low-and-Middle-Income-Countries-in-2019.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">cash from rich countries<\/a> other than the U.S. has been slumping ever since it peaked in 2014.<\/p>\n<p>And it\u2019s not just money, but activism, too. Red ribbons \u2014 the symbol of the fight against AIDS \u2014 are at risk of becoming pass\u00e9, as a younger generation devotes itself to green issues and Black Lives Matter.<\/p>\n<p>\u201cFundamentally, it\u2019s just not as sexy,\u201d said Robin Gorna, a longtime AIDS activist whose career evolved from protesting on the streets of London in the 1980s to running the International AIDS Society in 2010.<\/p>\n<h3><strong>It didn\u2019t used to be this way<\/strong><\/h3>\n<p>In the West, HIV strikes hardest among those who are already taking a beating from society: LGBTQ people, sex workers, drug users. <a href=\"https:\/\/www.politico.eu\/article\/telescope-is-curing-aids-worth-the-risk-vaccine-cure-hiv-clinical-trials\/\" rel=\"noreferrer noopener\" target=\"_blank\">There\u2019s no vaccine, and no cure<\/a>.<\/p>\n<p>And yet, the virus\u2019 early victims built a movement that became the envy of the patient world \u2014 a model for how people fighting a disease can also fight for better treatment.<\/p>\n<p>Gay men in San Francisco, New York and London may have faced severe stigma, but they were often well educated, wealthy and wise to the ways of politics and pop culture. (Larry Kramer, the notorious founder of ACT UP who <a href=\"https:\/\/www.nytimes.com\/2020\/05\/27\/us\/larry-kramer-dead.html\" rel=\"noreferrer noopener\" target=\"_blank\">died earlier this year<\/a>, was an award-winning, Yale-educated screenwriter whose scripts and investments made him a millionaire, for example.)<\/p>\n<p>Gorna, an idealistic feminist at Oxford in 1986, said she was swept up in the \u201cvibrancy and cheekiness and rage of these extraordinary gay men who were basically fighting for their lives.\u201d<\/p>\n<p>\u201cIt seems awful to say fun,\u201d she added. \u201cBut it was fun.\u201d<\/p>\n<p>In the late 1980s, Keith Haring\u2019s pop art put tolerance on T-shirts. Princess Diana <a href=\"https:\/\/www.bbc.com\/news\/av\/magazine-39490507\" rel=\"noreferrer noopener\" target=\"_blank\">shook hands<\/a> \u2014 sans gloves \u2014 with a dying man at the opening of the U.K.\u2019s first dedicated AIDS ward in 1987. In 1992, the <a href=\"https:\/\/www.dw.com\/en\/the-largest-farewell-the-freddie-mercury-tribute-concert-25-years-ago\/a-38487337\" rel=\"noreferrer noopener\" target=\"_blank\">Freddie Mercury AIDS Awareness Tribute Concert<\/a> introduced the red ribbon symbol to a billion people watching on TV.<\/p>\n<p>Soon, world leaders were paying attention. In 1996, UNAIDS became the first U.N. organization devoted to a specific disease \u2014 and it\u2019s still the only one. The U.N. Security Council recognized AIDS as a global threat in 2000. In 2003, then U.S. President George W. Bush, a staunch social conservative, launched PEPFAR, which helped deliver antiretroviral HIV medication to millions in Africa. The program is the largest commitment by any nation to fight a single disease in history, <a href=\"https:\/\/www.kff.org\/global-health-policy\/fact-sheet\/the-u-s-presidents-emergency-plan-for-aids-relief-pepfar\/\" rel=\"noreferrer noopener\" target=\"_blank\">according to the Kaiser Family Foundation<\/a>.<\/p>\n<p>The inflection point \u2014 the moment the tide started to recede \u2014 was the result of a stream of quiet medical breakthroughs in the mid 2000s. A new generation of antiretrovirals are increasingly tolerable for people to take every day \u2014 gone are the 22-pill regimens \u2014 allowing them to live with HIV into old age without debilitating side effects.<\/p>\n<p>The implications of the good news were just sinking in when the 2008 financial crisis blew holes in government budgets across the world. Both were bad news for HIV fundraising.<\/p>\n<p>\u201cIt\u2019s become much more of a challenge, because the perception is that HIV and AIDS is over,\u201d said Anne Aslett, CEO of the Elton John AIDS Foundation. \u201cThere is a long pattern of creating huge amounts of awareness for a specific issue, but moving on before the job\u2019s done.\u201d<\/p>\n<p>Indeed, things aren\u2019t on track. The U.N. had set 2020 as the year the world was supposed to meet the so-called 90-90-90 goals: 90 percent of HIV positive people diagnosed; 90 of those on treatment; and 90 percent of those virally suppressed (meaning they can\u2019t pass the virus on to others).<\/p>\n<p>Mathematically, achieving that goal would work out to 73 percent of people living with HIV having successfully suppressed the virus. But at the end of 2019, <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2020\/september\/20200921_90-90-90\" rel=\"noreferrer noopener\" target=\"_blank\">that figure was just 59 percent<\/a>, according to UNAIDS, and disruptions from the coronavirus pandemic could wipe out progress.<\/p>\n<p>Not only is the fight far from over. Globally, progress has pretty much ground to a halt. In 2018, 1.7 million people were infected with HIV \u2014 and that number stayed the same in 2019, <a href=\"https:\/\/www.theglobalfund.org\/media\/10103\/corporate_2020resultsreport_report_en.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">according to the Global Fund<\/a>.<\/p>\n<p>Funding, too, has stagnated. The total contribution from wealthy nations to low- and middle-income countries is about the same in 2019 as it was a decade ago, <a href=\"http:\/\/files.kff.org\/attachment\/Donor-Government-Funding-for-HIV-in-Low-and-Middle-Income-Countries-in-2019.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">according to the Kaiser Family Foundation<\/a>, even though the number of people living with HIV in poorer countries has increased by 25 percent. More people surviving with the virus means more people needing financial and medical assistance.<\/p>\n<p>\u201cBecause there\u2019s no cure, there\u2019s no vaccine, the only way to really keep addressing HIV in a significant way is to get as many people as possible on antiretroviral therapy,\u201d said Jen Kates, Kaiser\u2019s Director of Global Health &amp; HIV Policy. \u201cIt requires a significant sustained effort for a long, long period of time.\u201d<\/p>\n<p>In the West, antiretrovirals taken as pre-exposure prophylaxis (PrEP) provides another reason for complacency. Capable of preventing people from contracting HIV, these are <a href=\"https:\/\/www.politico.eu\/article\/aids-european-state-of-play\/\" rel=\"noreferrer noopener\" target=\"_blank\">widely available<\/a> and reimbursed in countries like France, the U.K. and Germany, where they are targeted primarily at men who have sex with other men.<strong> <br \/><\/strong><\/p>\n<p>\u201cOut of sight, out of mind,\u201d said MEP Nicolae \u0218tef\u0103nu\u021b\u0103, who bluntly acknowledged during a <a href=\"https:\/\/www.politico.eu\/event\/telescope-closing-event\/\" rel=\"noreferrer noopener\" target=\"_blank\">POLITICO event<\/a> last week that HIV isn\u2019t on the radar in the European Parliament. His native Romania doesn\u2019t have a PrEP program.<\/p>\n<h3><strong>Broadening the appeal<\/strong><\/h3>\n<p>With the urgency largely gone, as the original generation of activists grow older, they\u2019re struggling to pass the torch. There aren\u2019t young people lining up to take it.<\/p>\n<p>It\u2019s not hard to see <a href=\"https:\/\/www.nytimes.com\/interactive\/2020\/04\/13\/t-magazine\/act-up-aids.html\" rel=\"noreferrer noopener\" target=\"_blank\">similarities<\/a> between ACT UP and the climate activist group <a href=\"https:\/\/rebellion.global\/\" rel=\"noreferrer noopener\" target=\"_blank\">Extinction Rebellion<\/a>, with their die-ins, confrontations with politicians and other disruptive protests. But the HIV movement has aged out of those types of tactics, Gorna said, in part because they aren\u2019t relevant to what\u2019s needed today.<\/p>\n<p>\u201cNow, the kind of activism that\u2019s needed is much more complex, and it\u2019s much less emotionally charged,\u201d she said.<\/p>\n<p>\u201cIt\u2019s much easier to <a href=\"https:\/\/www.macmillandictionary.com\/dictionary\/british\/toyi-toyi_1\" rel=\"noreferrer noopener\" target=\"_blank\">toyi-toyi<\/a> in the street and demand your drugs than get into a complex conversation about procurement systems and training of health care workers and monitoring service delivery,\u201d she added, referencing the South African style of street protest.<\/p>\n<p>And older activists aren\u2019t always welcome to new ways of thinking, Gorna said. For example, sexism and homophobia are still animating issues, but they look different from how they did in the 1980s. Today\u2019s LGBTQ vanguard embraces a view of gender fluidity that\u2019s sometimes a bridge too far for radicals of another era.<\/p>\n<p>\u201cWhat we\u2019re not always doing is understanding where young people\u2019s rage is directly located at the moment,\u201d Gorna said. \u201cIt takes some humility by older activists to say, \u2018Okay, their world\u2019s different. We did the bit in our world. We did well enough.\u2019\u201d<\/p>\n<p>Increasingly, the route to keeping AIDS charities relevant is to talk less specifically about AIDS.<\/p>\n<p>When the Terrence Higgins Trust, the U.K.\u2019s oldest AIDS charity, hired Alison McCants to work on fundraising in January, she was supposed to help with a big new push. The sector has a \u201cgrowing reliance\u201d on cash from donors rather than government appropriations, noted THT\u2019s <a href=\"https:\/\/www.tht.org.uk\/sites\/default\/files\/2018-10\/Terrence%20Higgins%20Trust%20annual%20report%202018.pdf\" rel=\"noreferrer noopener\" target=\"_blank\">annual report for 2018<\/a>. It flagged \u201clarge cuts to public sector funding\u201d as a threat that\u2019s likely to continue.<\/p>\n<p>So then comes the fundraising conundrum: Few cities have been more successful at stifling HIV transmission than London, and like other NGOs, \u201cwe\u2019re victims of our own success,\u201d McCants said.<\/p>\n<p>That was, of course, before COVID-19 made things even harder. The economic shock put pressure on donors\u2019 pocketbooks. Social distancing led to canceled charity events. With a predicted 40 percent cut in non-government donations for its AIDS efforts<strong>,<\/strong> the trust is pivoting from an \u201cambitious growth strategy to damage control,\u201d said McCants.<\/p>\n<p>Before the pandemic, the trust ran a series of focus groups seeking to understand what motivated their core supporters, who fall into three broad categories.<\/p>\n<p>One is, of course, the gay men over 45 who lived through the crisis \u2014 and likely lost friends and lovers.<\/p>\n<p>Another group is women in their mid-30s to mid-50s, worried about complacency in younger generations, including their own kids who are reaching sexual maturity \u2014 and as they, in many cases, dive back into the dating scene themselves, McCants sa<a href=\"https:\/\/www.politico.eu\/article\/the-hiv-fight-is-growing-old\/?utm_source=RSS_Feed&amp;utm_medium=RSS&amp;utm_campaign=RSS_Syndication\" rel=\"noreferrer noopener\" target=\"_blank\">Read More \u2013 Source<\/a><br \/>\n[contf]<br \/>\n[contfnew]<br \/>\n        <img decoding=\"async\" src=\"https:\/\/snap.md\/wp-content\/uploads\/2018\/03\/politico-logo.jpg\"\/><\/p>\n<h5><a href=\"https:\/\/www.politico.eu\/\" rel=\"noreferrer noopener\" target=\"_blank\">politico<\/a><\/h5>\n<p>[contfnewc]<br \/>\n[contfnewc]<\/p>\n","protected":false},"excerpt":{"rendered":"<p>This article is part of Telescope: The New ..<\/p>\n","protected":false},"author":1,"featured_media":151703,"comment_status":"closed","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[48],"tags":[],"class_list":["post-151702","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-health"],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.5 -
